Program Narrative:

Create a complete PHP Page

Read and follow all the requirements carefully
Comment after reading/copying the Case Study with your ICON #, if not an ICON use your email (Gmail)
Use my Title Header as shown in my Program Layout
For the Filename replace xxx with your ICON number, if not an ICON use 2010 instead.
Case study without a source code will not be accepted
Student who did not sign with their ICON # or email address (if not an ICON) at the end of the article will not be accepted

Declare an associative array (student) with the following fields: First Name, Last Name, Course Code, Gender, Age, and Address. (Please see layout for other screen outputs, supply your info as values).

Display all the values from your associative array (using while, list each)

Using associative array do the following:

Display Full Name

Display Department Description using the codes below:
1 – College of Business and Accountancy
2 – College of Computer Studies
3 – college of Education

Test for Gender
If it’s “M” or “m” echo “Male”
Otherwise echo “Female”

Display Address
Display your ICON number (if not an ICON display Student Number)
